combine present and past participle?

I am translating something into English and I have come across a difficulty. How can I eliminate the 2 that-clauses in the following phrase?

"the documentation that is handed over and that constitutes the basis of the invoice."

Can I use a present and a past participle instead of the that-clauses within the same structure?

The proffered document constituting the basis of the invoice....

remove the second 'that' to yield:

"the documentation that is handed over and constitutes the basis of the invoice."

"Which" sounds better to me than "that".
