Sex in the city

The title is "Sex and the city", but unless I pronounce it slowly so that "and" becomes "en", "end", a stressed "an" or a complete "and", I don't think I can distinguish between these:

Sex and the city
Sex in the city

Can you tell the difference, unless they are said carefully and a little slower?

No this is a common mix up... "And" and "in" both end up being pronounced as syllabic /n/.

in Serbocroatian ''Seks i grad'' (Sex and the city) sounds like ''Seksi grad'' (Sexy citty) LOL
