Remember, that this was set in the olden days. Just because titles such as Mr./Mrs. etc are starting to wane nowadays, doesn't mean that they weren't quite common terms of respect before.

In the southern parts of the US, it is still common to refer to people as Mr. or Miss plus their first name, as a sort of combination mark-of-respect/affection.
