It means five fingers.
Also: "give me five".
I remember Brian saying to ALF [http://en.wikipedia.org/wiki/ALF_(TV_series)]
: "give me four" 'cause Alf has four fingers.

"High" because the hand is extended up into the air, "five" because the palms are flattened with all five fingers extended, stiff and separated, the common method of expressing the number 5 nonverbally.
