I think this is a way of telling someone "I'm worried about your weird behaviour"

It depends on the context... Andy is probably right...

If someone is f'ed up, they may have just gotten beat up, they could be drunk or on drugs...

They may be insane or have a bad family life...

I'm sure I'm leaving out things, but it can mean all sorts of things.

I agree with Skippy. I haven't seen Departed so I don't know the context. In my previous reply I made a wild guess at the context.
